REFERRAL CONSENT

Where a Referrer is noted above, I/We hereby consent to the Referrer giving our name(s) and contact details to the Recipients, and any of their Representatives, within 5 business days after informing me/us that the Recipients are able to arrange a loan to purchase the goods noted above. The Referrer has only informed me/us that the Recipients are able to provide credit assistance and has not discussed any particular credit product. I/We further consent to the Recipients making contact with me/us within 10 business days after receiving the referral. I/We confirm that the Referrer has not charged me/us a fee for the referral and I/We have been informed that the Referrer may receive a commission or other benefit for or attributable to the referral which has been detailed below.

PRIVACY ACT CONSENT

The Recipient/s noted above collect from me/us (being each of the Applicants and Guarantors named in the application or any directors of companies named in the application) and from other persons described below, personal information about me/us, for the purposes of:

assessing the application for finance, the guarantee and any related insurance proposal;
sourcing and arranging finance and insurance; and
other activities relating to these activities.
For the same purposes, the Recipient/s may disclose this information to and collect further personal information about me/us from:

third parties identified in:
my/our application for finance or any related guarantee or related insurance proposal; or
applications for finance for a company of which I am a director or otherwise an interested party or any related guarantee or related insurance proposal;
guarantors or proposed guarantors identified in my/our application for finance;
one or more of the credit providers they use to arrange finance on my/our behalf (the “Credit Providers”).
I acknowledge that the Credit Providers to whom the Recipients may submit applications on my behalf and their website addresses are set out in the Schedule at the end of this document;
Credit Reporting Bodies, where I/we have authorised the Recipient/s to obtain a report on my/our behalf as set out in section 4 of this form below;
my/our car or finance broker; and
I/we acknowledge that where I/we have been resident in New Zealand the Recipients may also seek credit reports from relevant New Zealand credit reporting bodies in relation to my/our application. In this regard I/we consent to Recipients disclosing my/our personal information (other than Credit Eligibility Information) to an overseas recipient, located in New Zealand. This consent means that Recipients do not need to take such steps to ensure that overseas recipient does not breach the Australian Privacy Principles and if that overseas recipient handles the information in breach of the Australian Privacy Principles, Recipients will not be accountable for that breach and I/we will not be able to seek redress under the Privacy Act 1988 (Cth). I/we also acknowledge that even where I/we do not provide Recipients with my/our consent to this, Recipients may still disclose personal information about me/us to the overseas recipient. However, in those circumstances, Recipients will do so in accordance with their obligations under the Privacy Act 1988 (Cth). By signing this form below, I/we consent to this disclosure of my/our personal information to overseas recipients.
I/we acknowledge that where I/we have been resident in New Zealand the Recipients may also seek credit reports from relevant New Zealand credit reporting bodies in relation to my application.

For the same purposes, the Recipient/s may also disclose my/our personal information to:

suppliers of goods, mercantile agents, collection agencies, and insurers; and
the Recipient/s agents, contractors, external advisers and other service providers.
I/We agree that each of the Recipient/s may use and disclose my/our personal information to their related bodies corporate or to third party affiliates or partners and that the Recipients and their related bodies corporate and third party affiliates and partners may use and disclose this personal information to inform me/us about their products and services in the future by direct marketing. I understand that I can opt-out of receipt of such information if I/we contact the Recipients at the contact details set out below.

I/We acknowledge that if we do not provide the personal information requested, the Recipient/s or the Credit Providers may not be able to process my application for finance or insurance or assess whether to accept me as a guarantor.

I/We acknowledge that Credit Providers are required to collect certain personal information about me under the Anti-Money Laundering and Counter-Terrorism Financing Act 2006 (Cth).
I/We understand that the Recipient/s’ privacy policies, which include a Credit Policy (available from each Recipient/s’ Privacy Officer) contain information on how I/we are able to gain access to the personal information or Credit Eligibility Information held about me/us by the Recipient/s and seek correction of personal information, Credit Eligibility Information and Credit Information (subject to exceptions stated in the Privacy Act 1988 (Cth) (“the Privacy Act”)) and on how I/we may make a complaint about a breach of the Australian Privacy Principles set out in Division 3 of Part IIIA of the Privacy Act or the Credit Reporting Code of Conduct and how the Recipient/s will deal with my/our complaint.

I/We agree the Recipient/s may collect information about me/us including health information or information about my/our citizenship and country of birth, membership of a professional or trade association, membership of a trade union, criminal record or information about me/us where I/We are a politically exposed person under the Anti-Money Laundering and Counter-Terrorism Financing Act 2006 and use or disclose that information as necessary where I/We have applied for a product that requires that information.

I/We agree that, if we supply the Recipient/s or the Credit Providers with personal information about any third party (such as a referee), I/we will inform that third party of this fact. I/We acknowledge that the Credit Providers collect and use personal information about me/us for the following purposes:

assessing the application for finance, the guarantee or any related insurance proposal; and
(if the application is accepted) providing finance and ancillary services (such as insurance) to the applicant(s) and for the ongoing management of the finance, guarantees and ancillary services; and
collecting payments that are overdue in relation to the credit provided to me by the Credit Provider.
For those purposes, and subject to the Privacy Act, I/we consent to and agree that the Credit Providers may do all the following now and in the future:

Disclose my/our personal information to a credit reporting body and collect credit reports and information about my/our credit history and credit worthiness, including credit liabilities, repayments and defaults, from a credit reporting body (or a body that provides information on commercial activity and commercial credit worthiness) to assess my application for consumer credit or commercial credit or to assess whether to accept me as a guarantor. The relevant credit reporting bodies are likely to be either Veda Advantage (www.veda.com.au Telephone: 1300850211), Dun and Bradstreet (www.dnb.com.au Telephone: 1300734806) or Experian (www.experian.com.au Telephone: 1300784134).
Collect from and disclose to another credit provider named in the application for finance or in information otherwise obtained in relation to the application any personal information about me, including credit eligibility information about me/us in order to assess my application for consumer credit or commercial credit.
Collect from and disclose to any body which maintains a register of personal property securities (such as PPSR), any personal information about me/us, including any information about my/our creditworthiness, credit standing, credit history or credit capacity, the status of the finance and any default by me/us in accordance with the Privacy Act.
Collect from and disclose to any person or organisation identified in the application for finance, personal information about me/us.
Collect information about me/us from the Recipient/s including health information or information about my/our citizenship and country of birth, membership of a professional or trade association, membership of a trade union, criminal record or information about me/us where I/we are a politically exposed person under the Anti-Money Laundering and Counter-Terrorism Financing Act 2006 and use or disclose that information as necessary where I/we have applied for a product that requires that information.
Disclose to any guarantor or proposed guarantor of the application for finance, any information about the finance or personal information (including credit eligibility information) about me/us now or at any time in the future in order for the guarantor or proposed guarantor to consider whether to offer to act as guarantor in relation to the credit or to offer property as security for the credit.
Collect from and disclose personal information about me/us to the Credit Provider’s agents, contractors, external advisers as well as to suppliers of goods, mercantile agents, collection agencies and insurers, and my/our car or finance broker.
Disclose to the Recipient/s any credit decision made by the Credit Providers concerning my/our application for finance.
I/we acknowledge that each Credit Provider’s website will generally include information on how to contact the credit provider and how to obtain a copy of its privacy documents in a form that suits you (e.g. hard copy or email) and will generally also contain a statement about credit reporting, including information about the credit reporting bodies to which they may provide my/our personal information and that they may include my personal information in reports they provide to other credit providers and how:

I/we may obtain a copy of the policies about the management of credit related personal information of the Credit Providers and the credit reporting bodies;
if I/we fail to meet my/our payment obligations or commit a serious credit infringement, credit providers may be entitled to disclose this information to Credit Reporting Bodies; and
I/we may access my personal information (including credit eligibility information) held by the Credit Providers, and seek the correction of personal information (including credit eligibility information or Credit Information), complain about a breach of the Australian Privacy Principles or a failure to comply with Division 3 of Part IIIA of the Privacy Act or the Credit Reporting Code and how the Credit Providers will deal with such a complaint;
I/We have a right to request credit reporting bodies not to use my credit reporting information for the purposes of pre-screening of direct marketing and to not to use or disclose such information if I believe on reasonable grounds that I have been, or am likely to be, a victim of fraud ; and
I/We can request to have this statement made available in an alternative form – such as hard copy or by email.
I/we acknowledge that a Credit Provider who approves my/our application for finance will provide me/us with its own privacy disclosure statement and consent document should I/We decide to proceed with the offer for finance.

HOW TO CONTACT US

AUTHORISATION TO OBTAIN CREDIT REPORTS

I/We authorise the Recipients, who I/we acknowledge are assisting me/us to deal with credit reporting bodies, to make a request on my behalf to obtain my/our credit reporting information about my consumer and commercial credit worthiness from credit reporting bodies.
I/we acknowledge that in order for Credit Providers to verify my/our identity for the purposes of the Anti-Money Laundering and Counter-Terrorism Financing Act 2006 in connection with this application:

the Credit Providers may disclose any or all of the following personal information to a credit reporting body:
my/our names;
my/our residential addresses;
my/our dates of birth; and
the Credit Providers may request a credit reporting body to provide an assessment of whether the personal information disclosed matches (in whole or part) personal information contained in credit reporting information about me/us in the possession or control of the credit reporting body (the Assessment); and
the credit reporting body may prepare and provide to the Credit Providers the Assessment and may use the personal information about me/us and the names, residential addresses and dates or birth contained in credit reporting information of other individuals, for the purpose of preparing the Assessment,
and I/we consent to the Credit Providers making the above request to a credit reporting body and the disclosure of my/our personal information.

ACCEPTANCE OF THIS CONSENT

Notwithstanding the terms of any consent given in this Referral & Privacy Act Consent, nothing in this Referral & Privacy Act Consent permits the Recipients or the Credit Providers to use or disclose credit eligibility information other than in accordance with the requirements of the Privacy Act 1988.

I/We solemnly and sincerely declare that I/We have read and accept and give all of the consents set out in this Referral and Privacy Act Consent form. I/we acknowledge that if I do not wish to give any of the consents set out in this Referral and Privacy Consent form, I can indicate this by striking out the relevant consent above and acknowledge that, if I do so, the Recipients may not be able to arrange finance or related insurances for me.

Definitions

All terms defined in the Privacy Act 1988 have the same meaning when used in this Referral & Privacy Act Consent.

ABOUT THIS FINANCIAL SERVICES GUIDE
This Financial Services Guide (FSG) is designed to assist you to decide whether you wish to use any of the
services we provide. It contains information about how we and others are remunerated in relation to the
services, and about how you may access our internal and external dispute resolution procedures.
This guide is made up of:
Part 1 – containing information about us and the services we can provide to you; and
Part 2 – containing information relevant to each Australian Financial Services Licensee on whose behalf we
are authorised to act.
Please keep this document in a safe place for your future reference.
OTHER DISCLOSURE DOCUMENTATION
You may also receive one or more of a Statement of Advice (“SoA”) or General Advice Warning (“GAW”), and
a Product Disclosure Statement (“PDS”).
• A Statement of Advice contains information about any personal advice we give you so that you can make
an informed decision about whether or not you act upon it. Personal Advice is advice that takes into
account any one or more of your objectives, financial situation and needs. The Statement of Advice will
contain advice, the basis on which it is given and information about how we are remunerated, as well as
any associations or relationships which may have influenced the advice. A Statement of Advice must be
given at the same time, or as soon as practicable after, personal advice is provided.

• A General Advice Warning, if given, will warn you that any advice given is of a general nature and does
not take your personal needs, objectives or financial situation into account.

• If we make a recommendation that you acquire a particular financial product, or offer to issue, or arrange

REMUNERATION AND OTHER BENEFITS
You may be charged a fee for our services. We will tell you if this is the case and agree with you on the type
of fee to be charged, how it is to be calculated and when it is payable before we provide our service.
We may also receive a commission (or share of it) for our services from our licensees or the product issuer if
our licensee is not the product issuer.
In some cases we receive an upfront commission which is part of the total premium payable by you for the
product. It is a percentage of either the:
• Gross Written Premium (“GWP”) being the total amount of premium paid by the customer to the insurer in
respect of the insurance arranged, or

• Net Premium (“NP”) being GWP less stamp duty, fire services levy (where applicable), GST and any other
government charges, taxes, fees or levies.

The commission varies depending on the licensee, product issuer and product. Commission may be received
by us when we receive your premium or by the end of the month or such later time as agreed with the licensee
or product issuer. It is paid each time you buy or renew a policy and in some cases when you vary it.
We may receive all, or a share, of other forms of commission. These include profit share and volume bonus
commission. They are payments from the product issuer and/or licensee based, over an agreed period, on:
• the profit that they earn on certain business; and/or

• the volume of certain business we introduce to them, within an agreed period.

It may be a percentage of the profit (or levels of profit) or an agreed amount depending on the level of profit
achieved and may be paid monthly, quarterly, annually or as otherwise agreed.
Our representatives may also receive:

• an annual salary which may include additional remuneration based on performance against sales targets
and/or other performance criteria

• various soft dollar payments such as business related conferences, study trips, or other functions paid for
by the product issuers, our licensees or our principals.

Where a third party has referred you to us, we may share with them part of the amount of commission or fees
we receive in relation to the relevant product or service we provide to you or pay them an agreed referral fee.
Further details of the remuneration (including commission) and benefits we may receive that are particular to
each licensee are set out in PART 2.
IF YOU HAVE A COMPLAINT OR DISPUTE
If you have a complaint or dispute, whether or not we are able to resolve your problem ourselves, we are
obliged to promptly refer them to the relevant Licensee. All Licensees have internal dispute resolution
procedures and are required to be a member of an ASIC approved external dispute resolution service
(“EDRS”). If we or the Licensee are unable to resolve your complaint internally it will be referred to their EDRS
which may be able to resolve the dispute. Please refer to Parts 2 and 3 for further details.

OTHER REMUNERATION, COMMISSIONS AND LIMITS
As stated in Part 1 of this FSG, we may also receive a volume bonus or additional commission from AWN.
This may be a percentage of the retail price or an agreed dollar amount. These amounts maybe based on the
volume of business we generate, our achievement of agreed sales targets and/or market forces within the
industry. All these factors may vary from time to time. These amounts will be paid periodically and will not
exceed 10% of the total amount that you pay.
We may receive items and prizes from AWN’s Reward Program. These include corporate conferences, caps,
shirts, jackets, pens and other miscellaneous items.
We may also be provided with other benefits such as point of sale advertising and marketing materials to assist
in the sale, marketing and promotion of AWN products.

DISPUTE RESOLUTION
If you have a complaint or dispute we are obliged to promptly refer it to the relevant Licensee. AWN will have
an internal dispute resolution procedure and is a member of an ASIC approved external dispute resolution
service (“EDRS”).
AWN is a member of Financial Ombudsman Service Ltd (FOS), an EDRS.
If AWN is unable to resolve your complaint you can contact FOS on 1300 780 808 and they will consider your
complaint. This is a free service.

(and their employees) in relation to the life insurance component of an Avea CCI product, see below.
National Insurance Choice (ABN 85 114 352 680) or Motorsure Pty Ltd (ABN 14 086 238 557) act as a The
Third Party Introducer of Avea’s products to us and provides these services under authority from Avea.
What remuneration is payable in relation to the financial services we provide?
The following sets out further details of the relevant remuneration payable by the licensee in relation to the
provision of our financial services.
AVEA AND ITS EMPLOYEES
Avea receives the premium you agree to pay for the product which may include amounts in relation to
stamp duty, GST, fire services levy and/or other government charges, taxes, fees or levies (where
relevant). Avea employees receive a salary from Avea and may receive bonuses, and incentives.
THIRD PARTY INTRODUCER (TPI), AUTHORISED REPRESENTATIVE(S) AND OTHER PARTIES THEY
CONTRACT WITH (AND THEIR EMPLOYEES)
Avea retains an agreed amount of the premium dependent on factors such as the type of product and
coverage and the relevant TPI’s services and retains the amount of premium received in relation to
relevant taxes, charges and levies. The TPI receives the difference between that amount and the
premium paid.
In addition to other parties that contract with the TPI to undertake some or all of the TPI’s duties(where
relevant), we will receive a share of the amount that the TPI receives from Avea, in the form of an agreed
commission and/or fee between the TPI and us and/or other party (as relevant).
Employees of the TPI and/or the Authorised Representative(s) (including directors and subcontractors) may
be paid a salary for their services and may also receive bonuses based on the volume of sales of all financial
products over a period and may participate in profit sharing arrangements, business related conferences, study
trips or other functions. The TPI and/or Authorised Representatives may share or pass on a proportion of any
additional remuneration or other benefits they receive to their authorised staff.

From time to time we may participate is sales incentive schemes and/or competitions and may receive
promotional items or financial assistance to assist with the promotion of Allianz and Allianz Life products. The
benefits received from these sales incentive schemes and/or competitions will not exceed 5% of the combined
annual Pleasurecraft Insurance and Pleasurecraft Equity Insurance net premium.
Our representative may share in our commission and/or be paid a salary. They may receive bonuses or other
incentives and rewards depending on their performance relating to the sale of products and other business
criteria.
DISPUTE RESOLUTION
If you have a complaint or dispute we are obliged to promptly refer it to the relevant Licensee. Club Marine will
have an internal dispute resolution procedure and is a member of an ASIC approved external dispute resolution
service (“EDRS”).
Club Marine is a member of Financial Ombudsman Service Ltd (FOS), an EDRS.
If Club Marine is unable to resolve your complaint you can contact FOS on 1300 780 808 and they will consider

MTA Insurance is given a binding authority to arrange the issue of insurance policies on behalf of both AAI
Limited and Suncorp which means it can approve and accept an application for insurance, it also handles
general insurance claims on behalf of AAI Limited. In providing any services under the binder, MTA act on
behalf of AAI Limited and Suncorp and not on your behalf.
The Corporations Act (Cth) 2001 requires Australian Financial Services Licensees to have arrangements for
compensating retail clients for losses they suffer as a result of a breach by the licensee or its representatives
of Chapter 7 of this Act, unless an exemption applies.
MTA Insurance has compensation arrangements in place.

HOW WE ARE PAID
The following sets out further details of the relevant remuneration payable by the licensee in relation to the
provision of financial services.

Equity Plus Insurance/Cash Benefit Insurance:
Our employee, contractor, director or associates who arrange insurance may receive a benefit paid monthly
ranging from 2.89% to 7.19% of the total amount you pay (including premium, GST and govt. charges). This
benefit may be taken in a number of ways including non-cash benefits to an equal value.
We may also receive a bonus which is based on the volume of business we generate and achievement of
sales targets. These factors may vary from time to time. The bonus is paid periodically and will not exceed
13.8% of the premium (excludes Consumer Credit Insurance (CCI)).
We may also participate in sales incentive schemes, competitions or promotions from time to time or on an
ongoing basis.

Remuneration and other benefits
We receive commission from our licensee. The commission is a percentage of the premium paid by you, less
any taxes or government charges and is detailed in this FSG. Commission may also be paid when you
renew or vary your insurance. If a policy has been financed, the finance provider may also receive a
commission from the licensee which is at no extra cost to you.
Where a third party has referred you to us, we may share with them a part of the commission we earn. Any
commission we pay to a referrer is at no extra cost to you.
We may also charge a fee for our services to you. Any such fee we charge is an additional cost to you and is
detailed in this FSG.
Later in this FSG it sets out more detailed information regarding our remuneration, including commission,

Our staff who provide the authorised financial services are paid a salary for their services and may also
receive bonuses based on the volume of sales of all financial products over a period. Swann may provide

As a licensed credit assistance provider, we are required to give you a Credit Guide as soon as practicable after it
becomes apparent to us that we may provide assistance to you in relation to a credit contract.
The guide includes information about us, our responsible lending obligations, the names of the credit providers with whom
we conduct most of our business with, our fees, charges, and any commissions we may receive, and our dispute resolution
process.
OUR OBLIGATIONS BEFORE PROVIDING CREDIT TO YOU
Under the National Consumer Credit Protection Act (NCCP), we cannot provide credit assistance to you by:

suggesting that you apply for a particular credit contract with a particular credit provider;
suggesting that you apply for an increase to the credit limit of a particular credit contract with a particular credit
provider; or
assisting you to do so, if the contract will be unsuitable for you.

COMMISSIONS
When you enter into a credit contract, we are likely to receive a commission, directly or indirectly, from the credit provider in
relation to the credit contract for which we have provided credit assistance. We have also entered into a volume bonus
arrangement with each of the credit providers with whom we conduct business, either directly or indirectly, and may receive
additional commissions depending on the total volume of business that we arrange with each of the credit providers. The
volume bonus arrangements are payable by the credit provider to us as the licensee.
You may obtain additional information from us about the fees and charges payable by you, the commission likely to be
received by us or the volume bonus arrangements in place, on request. This includes obtaining information about how fees
and charges payable by you are worked out and a reasonable estimate of the commissions likely to be received by us and
how the commission is worked out.
PAYMENTS TO THIRD PARTIES
A commission is likely to be paid to a third party for the introduction of credit business or business proposed to be financed
by the credit contract or consumer lease if credit assistance is provided through one of the following classes of persons:

a Licensed Finance Broker;
a Credit Representative of ours; or
a Referrer (usually being the supplier of the goods).

You may obtain from us a reasonable estimate of the amount of commission and how it is worked out on request.
HOW WE RESOLVE DISPUTES
We welcome every opportunity to resolve any concerns you may have with our products or service. If you have a
complaint, are not happy with our service or just want more information, you should tell us. By telling us, you give us the
best possible opportunity of fixing things. In the first instance, contact our Customer Service Team by calling (07) 3908
0800. If you are not satisfied with their response you can request that their manager address your concern.
INTERNAL DISPUTE RESOLUTION
If your concern is still not resolved to your satisfaction please write to our Internal Dispute Resolution Committee at United
Financial Services (Qld) Pty. Limited Po Box 7220 HEMMANT QLD 4174. Your concern will be reviewed by our IDR
Committee, which has authority to deal with the matter.
We will inform you of the outcome within 45 days or receiving your dispute or complaint.
EXTERNAL DISPUTE RESOLUTION
If your concern still remains unresolved to your satisfaction you can direct your issue for further review to the Credit and
Investments Ombudsman Limited (CIO), an external review scheme to which we belong.
External dispute resolution is a free service established to provide you with an independent mechanism to resolve your
specific complaints. However, you must attempt to resolve your complaint with us before contacting our external dispute
resolution scheme.

HOW WE RESOLVE DISPUTES
We welcome every opportunity to resolve any concerns you may have with our products or service. If you have a
complaint, are not happy with our service or just want more information, you should tell us. By telling us, you give us the
best possible opportunity of fixing things. In the first instance, contact our Customer Service Team by calling . If you are not
satisfied with their response you can request that their manager address your concern.
INTERNAL DISPUTE RESOLUTION
If your concern is still not resolved to your satisfaction please write to our Internal Dispute Resolution Committee at United Fortune Financial Pty Ltd. Your concern will be reviewed by our IDR Committee, which has authority to deal with the matter.
We will inform you of the outcome within 45 days or receiving your dispute or complaint.
EXTERNAL DISPUTE RESOLUTION
If your concern still remains unresolved to your satisfaction you can direct your issue for further review to the Credit and
Investments Ombudsman Limited (CIO), an external review scheme to which we belong.
External dispute resolution is a free service established to provide you with an independent mechanism to resolve your
specific complaints. However, you must attempt to resolve your complaint with us before contacting our external dispute
resolution scheme.
